Healthchek
Healthchek provides health care services through Ohio's Medicaid program. These services help Ohio's children get the care they need before a treatable illness becomes serious. Healthchek helps all children, teens and young adults from birth through age 20 who are eligible for Medicaid.
Prevention services
- A medical health history
- A complete exam
- A developmental screening
- A nutritional screening
- A vision check
- A hearing check
- A review of the child's immunization history
- A dental screening
- A lead poisoning test, depending on the age of the child
- A test for anemia and sickle cell anemia
- A tuberculin (TB) test
- A urine test
- A sexually transmitted disease (STD) test
- Health education
- Behavioral health and other rehabilitative services
How to qualify
- Must be eligible for Medicaid
- If not currently, enroll in Medicaid
