Healthchek

Healthchek provides health care services through Ohio's Medicaid program. These services help Ohio's children get the care they need before a treatable illness becomes serious. Healthchek helps all children, teens and young adults from birth through age 20 who are eligible for Medicaid.

Prevention services

• A medical health history
• A complete exam
• A developmental screening
• A nutritional screening
• A vision check
• A hearing check
• A review of the child's immunization history
• A dental screening
• A lead poisoning test, depending on the age of the child
• A test for anemia and sickle cell anemia
• A tuberculin (TB) test
• A urine test
• A sexually transmitted disease (STD) test
• Health education
• Behavioral health and other rehabilitative services

How to qualify

• Must be eligible for Medicaid
• If not currently, enroll in Medicaid
